Care Management

In partnership with the Allegheny County DHS/Area Agency on Aging, Lutheran Service Society's Care Management team helps seniors remain safe, healthy, and independent in their homes by coordinating essential services including:

• comprehensive in-home assessment
• adult day care
• respite care
• personal care
• chore service
• companions

This program exists to support the consumer's highest level of functioning by coordinating the most appropriate community-based services available to that person. Eligibility is based on need and determined by an assessment.

Please note: To request Care Management services in Allegheny County, you must call the Allegheny County DHS/Area Agency on Aging Senior Line at (412) 350-5460.
